IF THE PHOTO IS OF A CHILD UNDER THE AGE OF 13.
"I verify that I am the legal parent/guardian of the child listed below. This child is under the age of 13. I grant permission for the child listed below to submit his/her entry to have his/her photo displayed on froggyradio.com/nascarphotos, described herein."

1. YOUR FULL NAME
2. YOUR FULL ADDRESS
3. YOUR TELEPHONE NUMBER
4. YOUR RELATIONSHIP TO THE CHILD UNDER THE AGE OF 13.
